Eyes On You CONSUMER HEALTH DATA PRIVACY POLICY

This policy describes how we collect, use, and share "Consumer Health Data" as defined by the Washington My Health My Data Act. This policy is separate from our HIPAA Notice of Privacy Practices and applies to Washington consumers.

  • Health-Related Interventions: Information about surgeries, procedures, or vision therapy you have received or are seeking.

  • Prescription Information: Data related to your use of prescribed corrective lenses or medications.

  • Precise Location Information: We may collect your zip code or location data to help you find our clinic or provide directions, which could indicate your attempt to receive health services.

  • Other Health Data: Any information you voluntarily provide in "comments" or "message" fields on our website regarding your symptoms or health status.


2. Purpose of Collection and Use

We collect and use this data strictly for the following purposes:

  • Providing Services: To schedule appointments, respond to your inquiries, and provide the optometric services you request.

  • Communication: To send you administrative updates, appointment reminders, and requested health information.

  • Security & Compliance: To detect and prevent fraud, protect the security of our website, and comply with Washington state legal obligations.


3. Sources of Data

  • We collect Consumer Health Data directly from you when you:

  • Fill out a "Contact Us" or "Appointment Request" form.

  • Use our online patient portal.

  • Interact with our website via cookies or IP address tracking that may infer health interests.


4. Sharing of Consumer Health Data

We do not sell your consumer health data. We only share your data:
